REFERRAL FROM A GENERAL PRACTITIONER (GP) TO THE SOKE
The Soke is a leading provider of cognitive health care and rated Outstanding by the Care Quality Commission.
General Practitioners are able to offer their patients preferential access to The Soke’s multi-disciplinary team of psychiatrists, psychologists, therapists, coaches and other behavioural experts.
The Soke takes a holistic approach to the full spectrum of mental health needs including the treatment of depression, anxiety, family and couples’ therapies, eating disorders, compulsive behaviours, children & adolescent mental health issues and personal development coaching.
For General Practitioners this means that your patients will benefit from the experience and knowledge of a range of mental health experts, collaborating to ensure that desired outcomes are achieved as efficiently and seamlessly as possible.
The Soke’s Practice Model
Contracted practitioners
In order to protect the interests of clients and to create and open and collaborative working environment for our practitioners, The Soke operates under a contract model that does not rely on a remuneration-per-client basis. This enables the team to focus solely on the needs of those who come under their care, without the necessity to consider their own interests in tandem with the client’s recommended treatment pathway.
Multi-disciplinary team meetings
The Soke’s multi-disciplinary team meet daily to share best practice, take advantage of the colleagues’ expertise and cross-refer to condition-specific specialists, when appropriate. It is at this point that complex cases can be escalated to the supervision of a consultant psychiatrist, if needed. The daily meeting is also attended by a member of the Client Services Team who represents the client viewpoint whilst also ensuring that a clinical record of each meeting is maintained for governance purposes.
Clinical oversight
The Soke’s clear Governance Structure creates a system of accountability and ownership within our divisions of practice: Children & Families, Adults, Schools, Corporates. Each division and its Head of Department works under the supervision of a Consultant Psychiatrist who manages Clinical Risk, supervised by a Clinical Board, in turn reporting to the Management Board, regulated by the Care Quality Commission.

Exclusion Criteria
Whilst every case is reviewed with the help of the multi-disciplinary team (MDT), the following are consistently applied exclusion criteria:
Suicidal intent (as opposed to suicidal ideation)
Acute psychosis
Active schizophrenia
Dementia & neuro-degenerative conditions (advance stage)
Body Mass Index under 16 (with active eating disorder)
THE REFERRAL PROCESS

Email your referral to us, specifying whether you wish to be the initial point of contact in relation to your referral or whether you would like The Soke to contact your patient directly.
We will acknowledge receipt of your email within two hours.
You are also welcome to make a referral by calling +44 (0)20 7139 5051 and speak to a member of The Soke’s Client Services Team, providing an overview of the patient you wish to refer.

If you have emailed your referral, one of the Client Services Team will contact you or your patient (depending on your indicated preference, see step one) to discuss the presentation, talk about which services or practitioners may be suited to your patient and offer appointment and practitioner options.
If you have put The Soke in direct contact with your patient, you will receive an update from us once they have secured an appointment with one of our practitioners.

Your patient attends their appointment and determines whether they wish to continue with treatment, having discussed and received some guidance on outcome goals with their practitioner.
As a referring GP you will receive an update on the Treatment Pathway agreed between patient and practitioner pending client consent.

Monday to Friday: 8.30am - 6pm (excluding Bank Holidays)

If a patient requires urgent mental health support out of hours, please refer them to their local A&E.

If a patient is having a mental health emergency, they should be referred to their nearest A&E.
The Soke is not able to treat/take very acute mental health episodes/emegencies.

The Soke does not provide any inpatient care. Some clinicians at The Soke do have admitting rights at acute inpatient mental health providers. General Practitioners are not able to refer patients to The Soke for inpatient mental health care.
